In-home care includes a wide range of services provided in the home, rather than in a hospital or care facility. It can allow a person with Alzheimer's or other dementia to stay in his or her own home- safely. It also can be of great assistance to family caregivers. Our in-home services provide non-medical help, such as assistance with daily living.
Here is an example of in-home services Dementia Care that we provide:
- Companion services- help with supervision, safety, recreational activities or visiting.
- Personal care services- assistance with bathing, dressing, toileting, eating, exercising or other personal care.
- Homemaker services- running all aspects of housekeeping, shopping or meal preparation.
Hospital Discharge & Emergency Services
Discharging patients from acute care is a complex process that is fraught with challenges. Over 35 million hospital discharges occur annually in the United States, and the cost of unplanned readmissions is 15 to 20 billion dollars annually. Our oversight and assistance with proper hospital discharge can negate the possibility of readmission.
At Sunlife Home care, we can provide a caregiver within 24 hours of discharge, transport your loved one home, pick up medications, do grocery shopping for a healthy diet, and help with transfers, ambulation, housekeeping, meal prep and other activities of daily living.
